Who We Are
The Hawaii Association of Case Managers was founded in 1987 by a group of social workers and nurses working as case managers for the elderly. The association today is a diverse group of members who help individuals and their families to improve the management of their needs in meeting the challenges of healthcare and the aging process.
We have a diverse group of members who work in areas such as Adult Day Care, Assisted Living Facilities, Nursing Homes, Money Management, Durable Medical Equipment, Private Case Management Agencies, Non-profit Case Management Agencies, Veterans, Foster Care, and Residential Care Homes.
Our Mission
To decrease fragmentation; to provide cohesive, comprehensive and cost effective services; to advocate on behalf of the individual and/or representative and abide by the ethical and professional standards.

Our Definition of Case Management
Case Management is a holistic, comprehensive, multidisciplinary approach to the planning and delivery of care and services to meet the needs of individuals with aging and dependency issues in a cost effective manner.
