Who We Are:
The Hawaii Association of Case Managers was founded in 1987 by a group of social workers and nurses working as case managers for the elderly. The association today is a diverse group of members who help individuals and their families to improve the management of their needs in meeting the challenges of healthcare and the aging process.
Our members work in areas such as Adult Day Care, Hospice, Managed Medicaid, Assisted Living Facilities, Nursing Homes, Durable Medical Equipment, Case Management Agencies, Veterans Services, Adult Foster Care, and Residential Care Homes.

Our Definition of Case Management
Case Management is a holistic, comprehensive, multidisciplinary approach to the planning and delivery of care and services to meet the needs of individuals with aging and dependency issues in a cost effective manner.
